How do I participate?
Complete the online registration form linked on the Join FORWARD page. We will contact you by email, mail or phone.
After you enroll, you will receive a research questionnaire every six months and a FORWARD Magazine every 3 months.
Once you consent to participate, you will be taken to our registration page. Here is what our registration page looks like:
