Are you going to send my doctor with information in the questionnaire?

If we ever send your information to your doctor, we always make sure that we have your consent (permission) first. Some doctors we have partnered with are able to see their patients’ information in order to have a better understanding of how their patients are progressing. However, you will always be informed and consented before the transfer of data. There is no personal information that we would send to your doctor that they wouldn’t already have.
